It's idiomatic speech, and it's how one uses it and to whom it is said that matters. It can be quite rude if said to the wrong person in the wrong situation, and those situtations abound. You could say it to friends you know well and it wouldn't be particularly rude, but absolutely never to a perfect stanger, and certainly not your teacher or your boss at work.
Knickers generally refers to women's panties. It is considered a sexist remark because it implies irrationality, overreaction and melodrama associated strictly with females. Say it to a macho male in England and you could end up with a punch in the nose for in effect calling him girlie.
The way it is applied is generally as an insult and put-down, but without technically using crude or offensive words.
