I've been a victim of mistaken identity myself due to shared IP pools and people using common proxies. In my case the problem was a lot closer to home than I suspected. I didn't know my son was actively posting on the same sites. So with that experience, we do try to consider all the other information we have access to and give people the benefit of the doubt when something looks suspicious. There is a lot more to it than IP addresses alone. IPs are only one avenue of information we use for assessment.
I wouldn't think the spam problem is exaggerated. Spammers were draining countless man hours on exFamily.org at one point -- precious time which could have been put to better use. Different boards are subject to different spam attacks, and have different vulnerabilities. As a techie, I can say that anti-spam engines really aren't the answer. It's just playing by spammers' rules in a cat-and-mouse game. There are other far more efficient methods that are painless and maintenance-free, and have so far put a 100% halt to spamming for us.
