In Spanish, Don is a title of deference but not for mafia heads. It is a word of respect given to older men, such as great grandfather. It comes to mean "a very important person" with relation to mafia only when it is used in that context. Otherwise it is simply outmost respect.
Who would not respect Don Corleone? :-)
