I looked at http://www.xfamily.org/images/3/37/NameChange_-_Berg_Lurana_Nolind.pdf
and the order changes the name of Jennifer Susan Smith to Lurana Nolind Berg (not the other way around). It's quite possible that Lurana Nolind Berg did change her name to Jennifer Susan Smith and then changed it back.
